Enrolment takes one form.
How to enrol
Download and complete the enrolment form, then bring it to reception with photo identification and evidence of your eligibility to enrol: a New Zealand passport, birth certificate, or residence visa. Completed enrolment forms can also be emailed to reception@howickmedical.co.nz.
Transferring your records
Tell us your previous practice and we will arrange the transfer of your records. You will be removed from their register once the transfer completes.
Eligibility
To enrol you must be eligible for publicly funded health services in New Zealand. The enrolment form sets out the criteria in full.
